Residential Case Manager

Salary Range:  $20 - $20 hour

Child Crisis Arizona has been helping our community’s most vulnerable children, youth, and families for nearly five decades, offering help in times of crisis but also in non-crisis times to strengthen families and build resiliency. Our mission is to provide children and youth in Arizona a safe environment, free from abuse and neglect by creating strong and successful families. 

Our residential programs are looking to hire a Case Manager to join the team. As a Case Manager, you will be primarily responsible for the coordination of care of residents residing at Child Crisis Arizona.

ESSENTIAL FUNCTIONS

  • Participates in all aspects of programming.
  • Supports adolescents in engaging in meaningful activities to include employment, education, volunteer, extracurricular, community, etc.
  • Meets individually with clients to assess progress and needs.
  • Determines clients’ needs by completing intakes; determining need for medical, psycho-social, and psychiatric evaluations; reviewing evaluations, treatment objectives, and plans.
  • Maintains clients’ records by reviewing case notes; assessments, service plans logging events and progress etc.
  • Attend Child Family Team Meetings (CFT) Team decision Meetings (TDM) court hearing as needed
  • Prepares clients’ discharge by reviewing and amplifying discharge plans; coordinating discharge and post-discharge requirements and providing resources.
  • Establishes and maintains positive relationships with all adolescent, and maintains awareness of each adolescent’s status at all times.
  • Establishes and maintains positive and professional relationships with staff and outside contacts.
  • Maintains appropriate and complete documentation as needed.
  • Interacting professionally with other parties such as Child Protective Services Case Worker, parents, guardians, and other involved parties including Psychiatrist, Psychologists, RBHA, Shelter Aide, Parent Aides, CASA, Tribal Social Services, etc.
  • Coordinates visitation/contact of adolescent with family members and others as deemed appropriate.
  • Coordinates appointments including medical, specialty medical and others related to adolescent’s welfare as assigned.
  • Coordinates or provides transportation for appointments as assigned.
  • Attends quarterly team meetings, MDT’s and other meetings as scheduled by supervisor.
  • Assists in accessing and coordinating needed services for adolescent.
  • Answers Intake Calls and determines if it is an appropriate placement for the program.
  • Completes chart review weekly on all adolescent.
  • Seeks out and utilizes consultation services from other Agency staff as appropriate.
  • Coordinates the emotional and social needs of a child with staff.
  • Plans visits and referral appointments with family and accompanying as appropriate.
  • Advocates for adolescent and families, as appropriate.
  • Works with adolescent, individually and in small groups, to meet therapeutic goals.
  • Creates support plans that accurately capture the strengths, needs and culture of the child and/or family.
  • Follows Agency guidelines and policies.
  • Attends and participates in Agency related meetings and trainings as appropriate.
  • Participates in continuing education and training especially related to the field of child abuse and neglect.
  • Models Center’s expectation for sensitivity and competency relating to diversity and cultural awareness.

EDUCATION/EXPERIENCE

  • Bachelor’s degree
  • One year of full-time experience in case management.
  • Must have a valid Arizona Driver’s License
  • Must be at least 21 years of age
  • Must possess or be able to obtain a level one fingerprint clearance card
  • Must have experience in word processing in a Windows environment.
  • Must be able to display verbal and written communication skills that meet the requirements of the position and needs of clients.
  • Experience working with adolescent in the DCS system preferred.
